Survivorship Program Kit Nearing Completion

For the past two years we've been working on a survivorship program kit. The idea behind the kit was that it would contain everything a community cancer center would need to start a survivorship program. The kit contains the following elements:

1) The Institute of Medicine video From Cancer Patient to Cancer Survivor: Lost in Transition
2) Brochure that introduces patients completing treatment to survivorship as the next phase of care
3) Survivorship video to show to patients completing treatment
4) PowerPoint presentations to educate cancer center staff, hospital administration, and community providers about survivorship
5) Needs Assessment to document the problems experienced by patients during treatment
6) Treatment Summary and Care Plan Template
7) Survivorship Class syllabus and PowerPoint
8) Patient handouts on 79 survivorship topics
9) Recommendations for Survivorship Workshops for community survivors
10) Cancer Rehab program outline, policies, and forms
11) Annotated Bibliography of key survivorship publications
12) List of free survivorship publications and online resources
13) Survivors Day Celebration Planning Guide
14) Budget in Good, Better, Best format
15) List of potential external funding sources
16) Evaluation plan
17) Program brochure that describes the importance of providing survivorship services to our patients, along with instructions for unpacking the kit.

The kit will be shared with the other community cancer centers in our region during the month of December. Our plan is to make the kit available to community cancer centers beyond our region who need assistance implementing a survivorship program. If this plan is approved, the kit will be available through a website that we'll announce on this blog. Proceeds will go toward funding survivorship initiatives at our facility.
