Care Plan Template

We'll post our survivorship tools using Google docs (http://www.docs.google.com/). The care plan we chose was based on the Prescription for Living Care Plan [Haylock, P.J., Mitchell, S.A., Cox, T., Temple, S.V., Curtiss, C.P. (2007). The cancer survivor's prescription for living. AJN, 107(4), 58-70]. We modified it to include a more detailed late effects section, expand the patient reported toxicities and needs at end of treatment, add the associated distress caused by each toxicity or need, and improve readability, based on feedback from community physicians. We chose to rename it Prescription for LIFE (Live It Fearlessly Everyday) and cite the original authors in a footer. Here's a link to the template and a link to an example of a completed care plan.
